Dose and Duration

Neuromuscular blockade

Adult and child over 1 month: initially, 0.08–0.1 mg /kg by IV injection, then maintenance dose of 0.02–0.03 mg/kg (max 0.1 mg/kg in caesarean section) by IV injection or 0.8–1.4 μg/kg/minute by IV infusion, adjusted according to response.

Neonate: 0.08 mg/kg by IV injection, then maintenance dose of 0.03–0.05 mg/kg by IV injection or 0.8–1.4 μg/kg/minute adjusted according to response.

Preparation

  • Reconstitute each vial with 5 ml water for injections to give 2 mg/ml solution; alternatively reconstitute with up to 10 ml glucose 5% or sodium chloride 0.9% or water for injections.
  • Solutions reconstituted without water for injections are unsuitable for further dilution.
  • For continuous intravenous infusion, dilute to a concentration up to 40 micrograms/ml.

Contraindications

  • Hypersensitivity to rocuronium or bromide

  • Changes in vital signs
  • Prolonged neuromuscular block
  • Anaphylactic and anaphylactoid reactions

Interactions

  • Aminoglycosides (effect of vecuronium enhanced)

Pregnancy

  • Can be used.

Breastfeeding

  • Can be used.

Storage

  • After reconstitution, store at 2–8°C for not ≥ 24 hours.

⚠️ Caution

  • Use with caution in significant renal failure and hepatic impairment.
  • Use with caution in patients with hypersensitivity to other neuromuscular blocking medicines.
  • Patients with burns may require higher doses due to resistance to vecuronium.
  • Use lower doses in hyperthermia and myasthenia gravis.
  • Calculate doses on basis of ideal body weight in obese patients.
  • Duration of action is shorter in children, therefore maintenance doses are required more frequently.
  • Vecuronium causes paralysis of respiratory muscles therefore respiratory support is required until adequate spontaneous respiration is restored.
