Dose and Duration

Induction of anaesthesia

Adult under 55 years and child over 17 years: by slow IV injection or infusion, 1.5–2.5 mg/kg at a rate of 20–40 mg every 10 seconds until response

Adult over 55 years or debilitated: 1–1.5 mg/kg at a rate of 20 mg every 10 seconds until response by slow IV injection or IV infusion

Child 1 month–17 years: 2.5–4 mg/kg adjusted according to age, weight and response

Maintenance anaesthesia

Adult and child over 17 years: by IV infusion, 4–12 mg/kg/hour. By slow IV injection, 25–50 mg repeated according to response

Child 1 month–17 years: by IV infusion, 9–15 mg/kg/hour adjusted according to weight, age and response

Elderly and debilitated: by IV infusion, 3–6 mg/kg/hour or 25–50 mg by slow IV injection

Pregnancy: max 6 mg/kg/hour

Sedation of ventilated patients in intensive care

Adult and child over 16 years: by IV infusion, 0.3–4 mg/kg/hour adjusted according to response

Induction of sedation for surgical and diagnostic procedures

Adult and child over 17 years: initially by slow IV injection over 1–5 minutes, 0.5– 1 mg/kg, dose and rate of administration adjusted according to desired level of sedation and response

Child 1 month–17 years: 1–2 mg/kg by slow IV injection over 1–5 minutes adjusted according to response

Maintenance of sedation for surgical and diagnostic procedures

Adult and child over 17 years: by IV infusion, 1.5–4.5 mg/kg/hour, dose and rate of administration adjusted according to desired level of sedation and response. If rapid increase in sedation is required, give an additional 10–20 mg by slow IV injection

Child 1 month–17 years: by IV infusion, 1.5–9 mg/kg/hour (if rapid sedation required, add by slow IV injection, max 1 mg/kg)

Contraindications

  • Children < 16 years of age for sedation in intensive care
  • Hypersensitivity to propofol

  • Transient apnoea
  • Hypotension
  • Bradycardia
  • Headache
  • Nausea, vomiting
  • Anaphylaxis

Interactions

  • Suxamethonium (increased risk of myocardial depression and bradycardia)

Pregnancy

  • Can be used

Storage

  • Store between 2–25°C. Do not freeze

⚠️ Caution

  • Use with extreme caution if used for surgery of the mouth, pharynx, larynx and in patients with acute circulatory failure or fixed cardiac output
  • Use with caution in cardiac impairment, respiratory impairment, hypovolemia, epilepsy, hypotension, raised intracranial pressure, renal and hepatic impairment
  • Monitor blood-lipid concentration if there is risk of fat overload or if sedation is longer than 3 days
  • May depress neonatal respiration if used during delivery. Do not use for obstetric anaesthesia unless clearly necessary
  • Patient should not breast-feed for 24 hours after use of propofol as it is excreted in breast milk in small quantities
  • Use of a microbiological filter during infusion is not recommended
