Dosage Forms

Tablet

50 mg

Uses

  • Moderate and severe hypertension (stage 2)
  • Chronic heart failure

Dose and Duration

Stage 2 hypertension (in combination with another antihypertensive)

Adult and child >50 kg: 50 mg once daily (25 mg in intravascular volume depletion) adjusted according to response up to a maximum of 100 mg once daily

Elderly over 75 years: 25 mg once daily

Child 6–18 years (body weight 20–50 kg): 0.7 mg/kg up to 25 mg once daily (max 50 mg once daily). Reduce dose in intravascular volume depletion

Chronic heart failure

Adult: 12.5 mg once daily increased at weekly intervals to maximum 150 mg daily if tolerated

Contraindications

  • Hypersensitivity
  • 2ⁿᵈ and 3ʳᵈ trimester of pregnancy
  • Severe hepatic impairment

Side Effects

  • Dizziness, malaise, fatigue, vertigo
  • Hypotension including orthostatic hypotension
  • Angioedema
  • Anaemia
  • Hyperkalaemia, increase in blood urea, serum creatinine and serum potassium, hypoglycaemia
  • Renal impairment

Interactions

  • Ciclosporin (increased risk of hyperkalaemia)
  • Diuretics (enhanced hypotensive effect)
  • Potassium sparing diuretics, aldosterone antagonists (increased risk of hyperkalaemia)
  • Lithium (increased plasma concentration of lithium)
  • Potassium salts (increased risk of hyperkalaemia)

Patient Instructions

  • Preferably take your medication in the morning

Breast-feeding

  • Do not use

⚠️ Caution

  • Use with caution in renal artery stenosis, aortic and mitral valve stenosis, hypertrophic cardiomyopathy, severe heart failure and in history of angioedema
  • Reduce dose in mild to moderate hepatic impairment. Not recommended in children with hepatic impairment
  • Avoid in patients with severe renal impairment
  • Monitor renal function regularly especially in dehydration
