Uses

  • Hypertensive emergencies (including during pregnancy)
  • Hypertension
  • Heart failure

Dose and Duration

Hypertensive emergencies (including during pregnancy)
Adult: 5‒10 mg diluted with 10 ml sodium chloride 0.9%, by slow IV injection over 20 minutes. Repeat after 20‒30 minutes. Check blood pressure regularly
Hypertension (with another antihypertensive)
Initially, 25 mg every 12 hours orally, increased if necessary to maximum 50 mg every 12 hours
Heart failure
Adult: 20 mg by slow IV injection over 20 minutes

Contraindications

  • Myocardial infarction
  • Severe tachycardia
  • High output heart failure
  • Myocardial insufficiency due to mechanical obstruction
  • Cor pulmonale

Side Effects

  • Tachycardia, palpitation, low blood pressure
  • Fluid retention
  • Stomach upset
  • Dizziness
  • Systemic lupus erythematosus after long term administration of 100 mg daily

Pregnancy

  • Can be used. Avoid during 1ˢᵗ and 2ⁿᵈ trimesters

Breast-feeding

  • Can be used

⚠️ Caution

  • Reduce dose in kidney or liver impairment
  • Oral hydralazine should only be initiated by a specialist
