Uses

  • Essential hypertension
  • Heart failure with impaired left ventricular systolic function

Dose and Duration

Hypertension
Adult: 8 mg once a day. Dose can be increased at intervals of 4 weeks if there is inadequate response, to 16 mg once a day (max 32 mg once a day). Usual maintenance dose 8 mg once a day
Child 6–18 years: initially 4 mg once a day increased to maximum dose of 8 mg in patients <50 kg and 16 mg in patients ≥ 50 kg
Intravascular volume depletion: initially, 4 mg once a day
Renal and mild to moderate hepatic impairment: initial ly, 4 mg once a day adjusted according to response
Heart failure
Adult: initially 4 mg once a day; doubled every 2 weeks to a target dose of 32 mg or the highest tolerated dose

Contraindications

  • Hypersensitivity to candesartan
  • 2nd and 3rd trimesters of pregnancy
  • Severe hepatic impairment and/or cholestasis
  • Children below 1 year of age
  • Primary hyperaldosteronism

Side Effects

  • Respiratory tract infections, cough, nasopharingitis, oropharyngeal pain
  • Dizziness, vertigo, headache
  • Rash
  • Sinus arrhythmia
  • Nasopharingitis, oropharyngeal pain
  • Pyrexia
  • Hyperkalaemia
  • Hypotension
  • Renal impairment

Interactions

  • Aliskiren (increased frequency of adverse effects: avoid concomitant use)
  • Ciclosporin, potassium sparing diuretics, aldosterone antagonists, potassium salts (increased risk of hyperkalaemia)
  • Lithium (increased plasma concentration of lithium)

  • Do not use

⚠️ Caution

  • Careful titration of doses and thorough monitoring of blood pressure is required during dialysis
  • Use with special caution in patients with haemodynamically relevant aortic or mitral valve stenosis, or obstructive hypertrophic cardiomyopathy, renal artery stenosis,
  • Can cause fetotoxicity and birth defects when used in pregnancy
