Uses

  • Moderate to severe hypertension(Stage 2)

Dose and Duration

Moderate to severe hypertension(Stage 2)
Adult: 25–100 mg daily

Contraindications

  • Coronary artery vasospasm (Prinzmetal’s angina)
  • Hypotension
  • Marked bradycardia
  • Sick sinus syndrome
  • Cardiogenic shock
  • Metabolic acidosis
  • Second and third degree atrioventricular block
  • Severe peripheral arterial disease
  • Uncontrolled heart failure
  • Severe asthma and severe chronic obstructive pulmonary disease
  • Untreated pheochromocytoma

Side Effects

  • Gastrointestinal disturbances, constipation
  • Bradycardia, cold extremities
  • Fatigue, sweating
  • Bronchospasm

Interactions

  • Alpha blockers (enhanced hypotensive effect)
  • Amiodarone (AV block and myocardial depression)
  • Epinephrine (severe hypertension)
  • Lidocaine (increased myocardial depression)
  • Nifedipine (enhanced hypotensive effect)
  • Procainamide (increased myocardial depression)
  • Verapamil (asystole, severe hypotension, heart failure)

Pregnancy

  • Do not use in 1st and 2nd trimesters

Breastfeeding

  • Can be used

⚠️ Caution

  • Atenolol should not be withdrawn abruptly
  • Reduce dose in renal failure
  • May mask the signs of hypoglycaemia and thyrotoxicosis
